What is a concrete form anchor?

A concrete form anchor is a fall protection tie-off point installed into formwork before a concrete deck pour. The anchor embeds in the cured concrete during the pour and provides personal fall arrest during finishing operations and subsequent work on the deck. The Super Anchor Form-It is the contractor standard for poured-in-place commercial concrete work in Canada.

How many Form-It anchors do I need per concrete pour?

Determined by deck area, crew size, and the project’s fall protection plan. Typical spacing on commercial decks is one anchor per worker per work zone, with adequate spacing for fall arrest geometry. For large pours, work with your fall protection specifier to lay out anchor positions before formwork goes in.

Is the Form-It anchor reusable?

No. Form-It is a single-pour embedded anchor — once the concrete cures around it, the anchor is permanent (or removed/cut flush after the project if not retained as a permanent tie-off). For reusable concrete deck anchors on cured concrete, specify the Concrete Super Bolt (1040) or Sub-Surface Anchor (SSA) lines.
